D’Fine247 collects personal information in order to deliver the best possible service. Individual health, financial, demographic and administrative information collected, received, and created by D’Fine247 is considered personal information.
The type of personal information we may collect can include (but is not limited to) name, address, date of birth, contact details, information relating to your payment schedule as outlined in your membership agreement and health information obtained during assessment. We obtain this information directly from you. If this information is not available D’Fine247 may not be able to deliver the outcomes associated with the agreement.
Use and Disclosure
Information is collected to assist in providing services and client care. Other areas where the information is used include quality reviews, funding, legal affairs, education, research, collection of fees and planning. Your personal information is only made available to those directly involved with those providing services to D’Fine247 or those organisations that we authorise to manage the information.
Access to your information is possible by contacting the D’Fine247 Head Office and making a written request. A reasonable administration fee may apply.
Security and Protection
Most information is held in our membership records. Paper records are preserved and protected. Records are stored securely in accordance with the provisions of the Commonwealth Privacy Act.
D’Fine247 makes all staff aware of the requirements of its privacy policy, and the need to respect the confidentiality of client information and the privacy of individuals. We regard any breach of privacy very seriously and will impose disciplinary action, including dismissal, if appropriate.

D’Fine247 takes special care to ensure that the personal information it holds is accurate and up to date. Personal records can be updated by clients calling D’Fine247 Head Office directly.
